Summary:
The St.
Charles Health System's Wound & Ostomy Services provides the highest level of medical care for problematic, hard-to-heal wounds as well as ostomy care including colostomy, ileostomy, urostomy and fistulas to both inpatient and outpatients.
POSITION Overview:
The RN Certified Wound Ostomy provides quality patient and family centered care as a member of the health care team in accordance with The St.
Charles Health System's mission, philosophy, policies, and procedures.
Applies standards for professional nursing practice and focuses clinical care to enhance the experience for patients, families, and all others.
As a nurse this position will oversee the work of other caregivers providing specific care to assigned patient(s) throughout the shift, as required under the scope of the RN licensure.
This position does not directly manage any other caregivers.
This nursing position will provide and oversee the provision of specific care to assigned patients throughout the shift, consistent with the scope of RN licensure.
ESSENTIAL FUNCTIONS AND DUTIES:
Maintain and demonstrate an in-depth knowledge of nursing principles, practices, standards, and techniques and apply this knowledge in accordance with The St.
Charles Health System's policies and work instructions and within laws and regulations governing nursing practice in the State of Oregon.
Responsible for wound and ostomy care and oversight for patients referred to the Wound & Ostomy department; including wound care assessment, implementation of wound care interventions, ostomy consultations, stoma site markings, consultation with physicians, communication and collaboration with members of the care team, and support organizational education activities promoting wound healing and ostomy care.
Assess the patient's condition and needs using critical thinking and clinical judgment:
set outcomes; implement appropriate nursing actions and collaborates with other disciplines to meet the patient's/family's physical, emotional, spiritual, social, and intellectual needs; and evaluate patient's progress.
Provide and maintain a safe environment for caregivers, patients and guests.
Demonstrate awareness of and supports St.
Charles Health System's departmental and operating unit goals through participation in continuous quality improvement and departmental activities.
Participate in activities that promote professional growth and development of self and others.
Communicate (both written and orally) effectively and maintain professional relationships with caregivers, medical staff, patients, and guests.

Responsible for managing the patient's experience by clarifying, integrating, and coordinating the roles of the interdisciplinary team.

Calculates dosages and administers medications accurately in accordance with the organization's policies and procedures for medication administration.

As required by RN licensure.

LICENSURE/CERTIFICATION/REGISTRATIONRequired:
Current valid Oregon RN licenseBoard Certified Wound Care or Ostomy Care Certification through ABWM or WOCNCB.
Valid Oregon driver's license and ability to meet St.
Charles Health System driving requirementsAHA Basic Life Support for Healthcare Provider certification.
