Nurse Manager (Home Based Primary Care)

The Home Based Primary Care (HBPC) Team Manager is located within the Rehab and Long Term Care Division. The incumbent supervises and directs the Interdisciplinary Team. The HBPC Team Manager reports to the HBPC Program Director. Functions include:
Supervise and evaluate day-to-day activities and performance of the members of the interdisciplinary team Plan and manage team staffing (including hiring/terminations) to meet patient care needs, to achieve cost-effective unit operations, and to meet productivity goals Collaborate with the interdisciplinary team to continuously improve productivity, efficiency, patient driven care, the patient care experience, and patient care outcomes Assure that standards of care and professional practice are maintained Assess, maintain, and improves competence and the professional role of staff members which includes providing staff development opportunities and facilitating evidence-based practice Ensure standardization of nursing processes and compliance with regulatory mandates Establish and maintain a shared decision-making approach to leadership through area-based councils Participate as an active member of the Nursing Professional Council Work Schedule:

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan reimbursement program. If selected for this position you must meet specific individual eligibility requirement and submit your application within four months of appointment. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience. Moderate lifting, 15 -44 pounds, light carrying, under 15 pound, use of fingers, both hands, walking (1 hour), standing (1 hour), both legs required, operate motor vehicle, ability for rapid mental and muscular coordinating simultaneously, visual at least 20/40 each eye, both eyes required, ability to distinguish shades of color, hearing aid permitted, at least 70 peripheral in horizontal, mental and emotional stability.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
